Birth & Death Records

Our vital registrars record every reported birth and death that occurs within the geographic boundaries of Union County. Birth and death records dating back to 1909 are available in our office for genealogical review. Certified copies of birth and death records can be requested online, by email, by mail, or by stopping into our office. 

How to Get a Birth or Death Certificate:

  • Online – Order and pay online through the State of Ohio’s secure online portal. You can choose whether to pick up certificates at our office or have them delivered to your home using standard US mail (no additional charge). Please note a $4 convenience fee and a credit card processing fee are applied when ordering online.

  • In-Person – Come into our office at 940 London Avenue, Suite 1100 Monday through Friday between 8 am and 4 pm. No appointment is necessary and the entire process typically takes less than 10 minutes.
  • Email/Fax – Download the request form and credit/debit card payment form below. Complete the forms and email both forms to vitalstats@uchd.net or fax both forms to 937.645.3047 to the attention of the Vital Registrar. We will mail the certificate to the address provided or make it available for pick up in our office.
  • Mail – Download the request form and credit/debit card payment form below. Complete the forms and mail completed forms to Union County Health Department, Attention: Vital Registrar, 940 London Avenue, Suite 1100, Marysville, OH 43040. A check made payable to the Union County Health Department may be submitted as payment in lieu of the credit/debit card payment form. The Union County Health Department is not responsible for lost or returned mail.

Birth Certificates $24.00 (additional copies $22.00 each if ordered in the same transaction)

Veterans Copies If a certified copy of a birth or death certificate is required by the Veterans Administration, one free certified copy of the record can be provided. Veteran’s copies must be requested using a veterans request form. The veterans request form must be signed by the Veterans Administration or funeral director who has knowledge of proof of service in the military.
Our apologies, but due to state requirements, we are NOT able to provide complimentary copies of birth or death certificates (see exception for veterans listed above).

Birth Certificate

The Union County Health Department can issue birth certificates for anyone born in the state of Ohio from 1909 to the present. A certified copy of a birth certificate is $24. Additional copies of the same birth certificate at the same point of purchase are $22 each. Our apologies, but due to state requirements, we are NOT able to provide complimentary copies of birth certificates.

When requesting a birth certificate you will need to provide the following information:

  • Mother’s first name and maiden name
  • Father’s first and last name
  • Date of birth for the individual
  • Full name and address of person requesting certificate

Death Certificate

Certified copies of death certificates are available only for individuals whose death occurred within Union County.  We do NOT have state-wide issuance of death certificates. A certified copy of a death certificate is $24. Additional copies of the same death certificate at the same point of purchase are $22 each. Our apologies, but due to state requirements, we are NOT able to provide complimentary copies of death certificates.

When requesting a death certificate you will need to provide the following information:

  • Deceased’s full name
  • Date of death

Birth Certificates

  • Our office issues certificates for births that occurred in Ohio from 1909 – present. 
  • For births outside the State of Ohio, please contact the state in which the event occurred. 
  • For Birth Records in Union county prior to 1909, contact the Union County Probate Court at 937.645.3029 ext. 3405.  For Birth records in any other county prior to 1909, contact the probate court of that county.
  • When requesting a birth certificate you will need to provide the following information:

New Baby Birth Certificates

  • Certificates for new births typically take up to 2 weeks to become available. 
    • The information must be received from the birthing hospital (or center) and uploaded into the State of Ohio records system.
    • Our vital registrars’ process orders within 2 business days once the information is available.

Death Certificates

  • Our office only issues certificates for deaths that occurred within the geographic limits of Union County from 1909 – present. 
  • For Union County residents whose death occurs outside of Union County, please contact the county’s office of vital statistics where the death occurred or the Ohio Department of Health. 
  • For Death records in Union County prior to 1909, contact the Union County Probate Court at 937.645.3029 ext. 3405.  For death records in any other county prior to 1909, contact the probate court of that county.
  • For deaths outside the State of Ohio, please contact the state in which the event occurred. 
  • Providing the year of death is mandatory for obtaining a death certificate.  If you do not know the exact date please enter the year of death preceded by 99/99 (example: 99/99/2000).
  • When requesting a death certificate you will need to provide the following information:
  • Deceased’s full name 
  • Date of death 

Social Security Numbers on Death Certificates

Per Ohio Revised Code (ORC) 3705.23(5): For the first five years after a decedent’s death, a decedent’s social security number shall not be included on a certified copy of the decedent’s death certificate unless that information is specifically requested to be on the certified copy.
