Community Health Assessment

The Union County Community Health Assessment compiles statistics from several data sources including a community wide survey. The assessment helps paint a picture of how factors and behaviors that shape health are affecting the people of Union County. Community Health Improvement Plan

The Union County Community Health Improvement Plan lays out strategies for positively impacting the health of the people of Union County. Priority areas are chosen by a committee of community partners and informed by the Union County Community Health Assessment. The plan strives to achieve the vision of Union County as the healthiest community in Ohio to live, work, and play.

Health Equity Report

The Union County Health Equity Report explores how different geographic areas of Union County are more at-risk for poor health based on factors such as education, income, and access to healthy food and medical care that is close to home.

Communicable Disease Report

In Ohio, nearly 90 diseases are required to be reported to public health officials as part of disease surveillance efforts. The Union County Communicable Disease Report is an annual summary of what diseases were reported and a five year trend.
