1909 to present

The Union County Health Department has on file, birth and death records for persons who were born or who died in Union County only from 1909 to the present.

If the person you are researching was born or died prior to 1909 please contact the Ohio Historical Society, by mail at 1982 Velma Ave, Columbus, Ohio 43211 or by phone at 614-297-2300. You can find their online archives and research tools here.

The Union County Health Department's office hours are from 8:00 am to 4:00 pm Monday through Friday. In most cases, requests made in person at our office, can be processed while you wait. Certified copies must be requested by regular mail or in person, with the appropriate fee included (see below). You may request Union County genealogical information by email. If copies (certified or uncertified) are needed, the cost will need to be mailed or brought in. In either case, all requests for information will be answered within ten working days. Please, limit email requests to ten searches per week.

Birth Certificates

Birth records only from 1909 to the present are on file in our office for persons born in Union County.

To order a birth record through the mail, please provide the following information:

	Person's full name. If married, a woman must provide her maiden name.
	Date of Birth
	Father's name
	Mother's name
	Check or money order
	The address to which the certificate should be returned.

If the person was born outside of Union County, but still in Ohio, contact the county in which the person was born, or the Ohio Department of Health (ODH). ODH houses birth records of people born in Ohio from Dec. 20, 1908 to the present. A link for genealogical information about each Ohio County.

Death Certificates

Death records only from 1909 to the present are on file in our office for persons who have died in Union County.

To order a death record through the mail, please provide the following information:

	Person's full name. If married, a woman must provide her maiden name.
	Date of Death
	Check or money order
	The address to which the certificate should be returned.

Copies

A certified copy of a certificate has a raised seal and the signature of the Local Registrar of Vital Statistics. The cost for a certified copy is ten dollars ($10.00).

An uncertified copy or plain white copy is available for twenty-five cents ($0.25).

To apply for a certificate by mail, follow the instructions listed above and mail the request along with the correct amount (check or money order) to:

If the person died outside Union County, and still in Ohio, contact the county in which the person died, or contact the Ohio Department of Health (ODH). ODH houses death records of people who died in Ohio from January 1, 1945 to the present.

Here are some reasons you may need a certified copy of a record:

Birth Certificates

	To apply for a new social security card
	To apply for a social security benefits
	For school enrollment
	For a minor to apply for a work permit
	To obtain a replacement driver's license or apply for a driver's license
	To apply for a passport
	For identification purposes
	For some areas of employment

Death Certificates

	To apply for insurance and other death benefits of the deceased
	To apply for a social security or veterans benefits for the deceased
	To file with probate court

We will search our files and allow you to view the documents related to your request. For a limited time, you may request information by using the form below or by email, at no charge. Please, limit email requests to ten searches per week. We will provide a response within ten working days.
