Birth Certificate
* Person's Full Name (including maiden name)
* Date of Birth
* Father's Name
* Mother's Name and Mother's Maiden name
Death Certificate
* Person's Full Name
* Date of Death
· For both birth and death certificates, a self-addressed stamped envelope (if requesting by mail) and a check or money order made out to the Union County Health Department are required. For credit card payments, please print and fill out this form and return it to the Health Department.
The charge for a copy of a birth/death certificate through the Union County Health Department is $22.00.
